Kole tang in trouble

My Kole tang is pretty much staying in one semi secluded are of the tank. I noticed yesterday he didn't come over to feed on the Nori when I put it in. He has a spot on both the right and left sides on the gill area and appears to have a distended belly. Here is a link to pictures.

Any ideas of what it might be?


180 gallon, 30 gallon sump, 3 250W MH + 4 ATI T5's, MTC MVX 36 Skimmer, Apex controller Reef Octopus CaRx

Current Tank Info: 2 Barred Rabbitfish, Yellow/Purple, McMaster Fairy, Red Fin, Hoevans, Possum, 2 Leopard Wrasses, Kole, Purple & Atlantic Blue Tangs, Matted Filefish, 2 Percula Clown, 4 PJ and2 Banggai Cardinalfish , Swallowtail and Bellus Angel,

100 gallon DT and 50 gallon sump with refugium. Reefbreeders Photon V2+.

Any other symptoms like flashing or heavy breathing? was it ever treated for flukes?
My kole tang does get some discoloration on it from swimming between the rocks and rubbing against them in the tank but in this case where its on both sides of the fish by the gills, could it from something irritating him in the gills and its scratching himself?

300g with velocity t4 for return and a Reflo dart on a closed loop, T5 lights & SRO3000 skimmer.

Current Tank Info: 300g
